Register online at www.eayso.org , or pick up a form at Trophies Unlimited in Cottonwood, or print (3) copies from our website under "forms" and mail them in.

Registration is open to children 4 yrs to 13 yrs.

(must turn 4 by March 1st)

Fees:  $55 1st child

                     $40 2nd child (same family)

                     $20 3rd child (same family)

We do NOT take requests for specific coaches and/or teams, so please don't ask.

*For soccer accident insurance coverage you must have a social security number or student visa number to be eligible.*
